The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 04/16/2019 regarding the center?s physical plant. The inspection was initiated on 05/23/2019 and completed on 05/23/2019. On 05/23/2019, the inspector was on site from 9:20am to 11:08am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Due to the nature of the complaint, the inspector referred the allegation to the appropriate agency. Though not a part of the original complaint, there were violations found during the inspection regarding the center?s physical plant. See the violation notice for more details. Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed several areas of the center with chipped and peeling paint. 2. There were several areas of the center with missing wood molding which exposed crumbling drywall and insulation. 3. The yellow birds classroom had a broken radiator cover which exposed sharp plastic. 4. There was a piece of children's furniture that had tears which exposed foam, presenting a choking hazard. 5. The inspector observed dirt, dust, cobwebs and dead insects collected in floor corners, ledges, and window sills. Plan of Correction: Maintenance has started repairs and expect repairs to be completed in 90 days.
